Continuing or returning F-1 visa students can use this form for any of the following purposes:
- Dependent I-20 Request
- Initial I-20 Request (returning to Lewis from authorized leave)
- This is only for continuing students who have been out of the United States for more than 5 months and wish to re-enter the U.S.
- If you are a new student who has not yet been admitted to Lewis and would like to enroll and obtain an I-20, please email international@lewisu.edu.
- Reinstatement I-20 Request
- I-20 with Travel Endorsement Request
- Updated I-20 Request (lost old copy, biographical, financial, or program updates)
